Routine Care Coordination
We provide assistance with doctor’s office visits, managing appointments, and coordinating care across multiple providers. Assistance with VNA Referrals, Hospice Referrals, Home Health Referrals.
In-Home Nursing Assessments
With a degree in Nursing, we can conduct non-skilled nursing assessments to help determine the best steps for your future care based on your ability to manage Activities of Daily Living (ADLs). Additionally, we offer cognitive assessments, including the MMSE (Mini-Mental State Examination), to establish a baseline of cognitive function.
Assisted Living Placement
With experience in director roles in Assisted Living facilities (including Memory Care Director, Nursing Director and Executive Director), we understand the importance of finding a safe, compliant living environment that meets all state requirements and standards. We’ll work with you to identify the best options that align with your needs and budget.
Dementia Education
We offer specialized education and expertise in dementia care, along with support for individuals affected by dementia and Alzheimer’s disease, as well as their families.
